More than 30 scientific articles on endothelin were published over the past 3 decades, leading to the development and subsequent regulatory approval of a new class of therapeutics—the endothelin receptor antagonists (ERAs). Macitentan is an endothelin receptor antagonist which blocks ET-1 from binding ET A and ET B receptors. Endothelin is responsible for pulmonary artery vasoconstriction and hypertrophy, contributing to the pathology of PAH.

Other endothelin receptor antagonists, particularly bosentan, have been associated with hepatoxocity. Mahajan, in Essentials of Neuroanesthesia, Endothelin-1 Receptor Antagonists.
